CCISDA is hosting a webinar introducing the Data Exchange Framework (DxF), which is a part of California Health and Human Services' Center for Data Insights and Innovation. While the Department of Health Care Services' CalAIM and Behavioral Health Transformation (BHT) initiatives are the drivers for data sharing, the DxF supports the 'rules of the road' for data sharing. DxF consultants will present a recently drafted report on the Capabilities for Health and Social Data Sharing (which includes a general model and county case studies). The intent of the report is to provide 'live' examples which may be helpful to other counties and communities as they seek to more efficiently coordinate care--especially in resource constrained times when building wholly new enterprise architectures may be cost prohibitive. The Data Exchange Framework program is eager to gain a stronger understanding of the County perspective as we seek to support an ecosystem of appropriate data sharing.

Sophia Chang, MD, MPH

Dr. Sophia Chang is a seasoned healthcare executive and clinician, with policy and operational experience in health technology, managed care, public insurance, primary care, population management and public health. She is currently an Informatics Consultant to California Health and Human Service’s Center for Data Insights and Innovation, which oversees the California Data Exchange Framework. She is the former Chief Clinical Informatics Officer at Clover Health, a Medicare Advantage plan, and was responsible for Clover's clinical technology platform.

Prior to Clover, Dr. Chang was Chief Clinical Innovation Officer at CareMore Health Plan. Her previous positions include: Vice President of Programs at the California HealthCare Foundation (CHCF), Vice President and Medical Director of population health solutions at Accretive Health, Director of the Veterans Health Administration's Center for Quality Management in Public Health, Medical Director of the San Francisco Health Plan (a Medicaid managed care plan), and Director of AIDS Health Services for the San Francisco Department of Public Health. Dr. Chang continues to practice general internal medicine at San Francisco General Hospital as a UCSF faculty member. She holds a BA from Amherst College; an MPH from the University of California, Berkeley; and an MD from Columbia University's College of Physicians and Surgeons. 

 

Mary-Sara Jones

Health and Human Services Business leader with 25 years’ experience transforming care through process redesign, innovative technology, and person-centered approaches. With deep expertise in health care and social programs, Mary-Sara fosters strategic relationships with state and local agencies, community-based organizations and health information technology providers to streamline processes and improve outcomes.

Throughout her career, Mary-Sara has focused on leveraging data and cross domain interoperability to improve access to care for vulnerable populations through the adoption of holistic models of care. In California, Mary-Sara led the design and implementation of coordinated care solutions for San Diego, Los Angeles, and Sonoma Counties.
